The following is a summary of information you need to know about me, my fees and services before beginning psychotherapy. This includes disclosures the State of Washington requires psychologists to make to prospective clients. Please read this information carefully and, if you choose to meet with me, ask me to explain anything you do not understand.

CREDENTIALS
I am a licensed Psychologist in the State of Washington (#PY 2064). Licensure ensures that the psychologist has a doctoral degree from an accredited university and has passed a national written examination as well as an oral examination given by the Washington State Examining Board of Psychology. I received the Ph.D. degree in clinical psychology from the University of Massachusetts, Amherst, in 1995. Additionally, I completed three years of clinical fellowship at Harvard Medical School, Boston, from 1993-96.

DEFINITION AND COURSE OF TREATMENT
Treatment is psychotherapy designed to meet the specific needs of an individual, couple, or family, and involves verbal dialogue between the psychologist and client. Initially we will focus on information gathering and problem assessment, to be followed by the definition of mutually agreed upon goals for treatment. Subsequent psychotherapy sessions will focus on working toward the defined treatment goals, which may be revised as part of that process. Progress is achieved usually via the development of insight and self-understanding related to the defined problems and goals. Treatment is ended gradually as the goals are reached. Clients are required to become active participants in all stages of treatment. The length of treatment will vary, and is usually determined by the client's goals, motivation, and resources.

CLIENT RIGHTS, RESPONSIBILITIES, CONFIDENTIALITY
Clients have the right to request a change of therapy, referral to another therapist, or to discontinue therapy at any time. Additionally, clients are responsible for choosing a treatment provider and type of therapy that best suit their needs. I encourage you to raise questions regarding therapeutic approaches and progress. You may bring up any questions or concerns you have about me, my services, fees or policies at any time. For major problems that you feel remain unresolved, or if you feel I have acted in an unethical or unprofessional manner, you may contact my licensure board at: Department of Health, Examining Board of Psychology, P.O. All client-psychologist communications are considered privileged, and may only be disclosed under specific circumstances such as when a client has requested release of the information and signed a valid disclosure authorization. Additionally, psychologists are required by law to break confidentiality under the following circumstances: If the psychologist learns that a child, adult dependent person, or a vulnerable adult is being abused or neglected; if the psychologist believes a client is a danger to himself, herself, or others, or is unable to meet basic needs essential to his or her health and safety (and no one else is providing for those needs); if the psychologist is being investigated for disciplinary matters.

OFFICE
Located on Greenwood Ave. in Phinney Ridge, I share this quiet, confidential space with two other psychotherapists. There is free parking at the office.

EMERGENCIES
If you have an emergency, call me at (206) 991-3710 and leave a voicemail message. I will be paged and will return your call as soon as possible. Please be aware that I am not always available to return your call immediately. If I do not return your call within the time your emergency requires, you should either call the crisis line in Seattle at (206) 461-3222, go to your nearest emergency room, or call 911.

FEES
The fee for the initial therapy appointment is $150. Each subsequent visit is $125. Payment is due upon receipt of service. In cases where I have a contract with an insurance company these fees are often lower. I see some clients at a reduced fee based on financial need. I will try to answer any questions you have about fees, but you may have to contact your insurance company for some questions. Separate fees apply for psychological testing and report writing. Fees for court appearances and other interactions with the legal system are higher, and range from $250 - $500 per hour.

CANCELLATION POLICY
Therapy appointments are 45 minutes in length. There is no charge for appointments cancelled with 24 hours notice. A fee of $75 is charged for missed appointments and appointments cancelled with less than 24 hours notice. Insurance does not cover this fee, so it is the client's responsibility to pay it. Payment of this fee is due at the beginning of the next scheduled session. Psychologists have the right to use collection agencies and small claims court to pursue delinquent accounts.

INSURANCE COVERAGE
If you use insurance for payment, I will provide billing for you. The amount not covered by your insurance (e.g. deductible, copay) is due at the time of service. Payment for services is your responsibility, so I ask you to keep track of billings and payments. If you have questions at any time, do not hesitate to ask me.

Most managed care companies require pre-authorization before seeing a psychologist, and it is the client's responsibility to obtain it. If you have not obtained pre-authorization, please tell me in your first visit. Some insurance companies require more detailed information, such as symptom lists or treatment summaries, in order to process claims. I will request your authorization to release the information necessary to bill your insurance company.

If you change insurance carriers, please notify me immediately. Do not assume that your new carrier will cover your treatment. If you have a choice of new carriers, you may want to check which ones recognize me as a provider and pay for my services.
